Family Emergency Preparedness Form

I've created this downloadable Emergency Preparedness Checklist for Families to prep for the very unlikely scenario that my husband and I would both be unable to care for our kids due to illness. Our teens, or anyone else stepping in, would have all the information they need to hold down the fort. It's a work in progress, so it may need a few tweaks or personalization, but will get you started.

You’ll also find an Authorization and Consent to Treat Minors* form at the end to allow others to authorize care for your kids if you can’t.
